EIGHTH SCHEDULESection 76(2).Particulars to be submitted by occupiers of premises (other than factories) in which a steam boiler or air receiver is used
1. Name of the occupier of the premises.
2. Postal address and telephone number, if any, of the occupier.
3. Precise location of the premises.
4. Nature of the work carried on in the premises.
5. The following particulars in respect of each steam boiler or air receiver in use:
type, description and distinctive number;
country and year of manufacture, where known;
date of the last thorough examination and name of the person by whom the examination was made;
maximum permissible working pressure in newtons per square metre.
