Section 4

Duty of witness

Before witnessing the execution of the directive on the prescribed form, a witness who is a medical practitioner must take reasonable steps in the circumstances to ensure that the patient —

(a)

is not mentally disordered;

(b)

has attained 21 years of age;

(c)

has made the directive voluntarily and without inducement or compulsion; and

(d)

has been informed of the nature and consequences of making the directive.
