Statement of affairs by company making application
(1)
The statement as to the affairs of the company that must accompany a company’s application under section 250D must be made in the relevant form specified on the designated website and must contain the following information and particulars as at the latest practicable date before the making of the application:
the particulars of the assets, debts and liabilities of the company;
the name and address of each creditor of the company;
the securities (if any) held by each creditor of the company;
the dates when the securities mentioned in paragraph (c) were respectively given;
any further information as may be prescribed.

The statement must be made by —
one or more of the persons who are, at the date of the application under section 250D, directors of the company; and (b)the secretary of the company.
