For the purposes of section 191 of the Act —(a)the notice of the liquidator’s appointment and of the address of the liquidator’s office mentioned in section 191(1)(a) of the Act must be in Form VWU‑4, and must contain the following information:(i)the name of the company being wound up;(ii)the name and identification number of the liquidator;(iii)the date of the liquidator’s appointment;(iv)the address of the liquidator’s office;(b)the notice of the change in the address of the liquidator’s office mentioned in section 191(1)(b) of the Act must be in Form VWU‑5, and must contain the following information:(i)the name of the company being wound up;(ii)the name and identification number of the liquidator;(iii)the new address of the liquidator’s office; and(c)the notice of the liquidator’s resignation or removal from office mentioned in section 191(3) of the Act must be in Form VWU‑6, and must contain the following information:(i)the name of the company being wound up; (ii)the name and identification number of the liquidator;(iii)the date of the liquidator’s resignation or removal from office, as the case may be.
