Particulars required for licence application
For the purposes of section 10(2)(c)(viii) of the Act, an applicant for the grant or renewal of a licence to provide an outpatient renal dialysis service must, where the applicant provides or intends to provide haemodialysis, state the number of haemodialysis stations used or to be used in the provision of the service.
