Prescribed forms under sections 42J(3) and 42K(3) of Act
Subregulation 1
For the purposes of section 42J(3) of the Act, the written authority must be in Form 1 in the Schedule.
Subregulation 2
For the purposes of section 42K(3) of the Act, the written authority must be in Form 2 in the Schedule.
