Regulation 25

Form of applications, etc.

Every application by a person —

(a)

for the withdrawal of money in the Fund for the payment of any premium under the Scheme;

(b)

to increase or reduce his or her insurance cover under the Scheme; (c)to be exempted from the Scheme; or

(d)

to terminate that person’s insurance cover under the Scheme,must be made in any form and supported by any evidence that the Board may require.
