Clinical laboratory service licensee — records to be kept for each test
A clinical laboratory service licensee must keep records of all of the following information in relation to each test of a specimen that the licensee conducts:
the date, time and type of the test conducted;
(aa)where the test was conducted at temporary premises that are residential premises — the address of those residential premises;
the name of the person who conducted the test;
the results of the test;
the name of the person who certified the test;
all data and workings, including instrument raw data and laboratory worksheets, relied upon in the issuance of the clinical test report.
