—(1) The record of the proceedings of a review committee is to include —(a)the name and particulars of every person who participated in the proceedings;(b)the name and particulars of every person who was requested to participate in the proceedings but failed to do so;(c)a summary of the evidence presented by or on behalf of each party to the proceedings in sufficient detail to enable the Council to comprehend the course of the proceedings;(d)the reason for any adjournment;(e)any other information relevant to the decision of the review committee;(f)the decision reached by the review committee and the reasons for its decision; and(g)the terms of any family exclusion order, family visit limit, third party exclusion order or third party visit limit made or varied.(2) The record of proceedings is to be signed by the chairperson of the review committee and all the other members of the committee.(3) A summary of the record of proceedings (including the decision reached by the review committee and the reasons for its decision) must be submitted to the Council.
