An application to amend the name of a licensee must be made by the licensee concerned no later than one month before the amendment is to take effect.

For the purposes of section 15(3)(b)(i) of the Act, the prescribed time is 10 calendar days before the date the licensee intends to stop providing the licensable healthcare service at the approved permanent premises, using the approved conveyance or by the service delivery mode, as the case may be.

For the purposes of section 15(3)(b)(ii) of the Act, the prescribed time is 10 calendar days before the date the licensee intends to stop providing the approved specified service.

An application to amend the particulars of a licence, other than the licence conditions and the matters mentioned in paragraph (1), (1A) or (1B), must be made by the licensee concerned no later than 10 calendar days before the amendment is to take effect.
