Declaration by master or surgeon of ship

For the purpose of section 28 of the Act, the master or surgeon of any ship arriving in Singapore shall make and sign an entry declaration according to the Form set out in the Fourth Schedule as to the number of crew and passengers, the presence or prevalence of any infectious disease on board during the voyage, the number of deaths and such other particulars as are required in that Form.
Subregulation 2
Such a declaration purporting to be signed by the master or surgeon shall be deemed to have been so signed.
