Form of application for judicial management order
Subregulation 1
Every application for a judicial management order must be made in Form CIR-3 supported by an affidavit in Form CIR-4.
Subregulation 2
The affidavit supporting an application for a judicial management order must be —
(a)
deposed to by the applicant or by any of the applicants (if more than one) or, in the case where the application is made by a corporation — by a director, secretary or other principal officer of the corporation; and
(b)
filed together with the application.
