Forms for OCPO or FRO made upon conviction
Subregulation 1
An OCPO under section 15(2) of the Act must be in Form 5 set out in the Schedule.
Subregulation 2
An FRO under section 21(2) of the Act must be in Form 6 set out in the Schedule.
