Regulation 13

Certificate of registration

Subregulation 1

Upon a medical practitioner being registered as a designated workplace doctor under this Part, the Commissioner shall —

(a)

issue him with a certificate of registration; and

(b)

record his name and relevant particulars in the register of designated workplace doctors.

Subregulation 2

A certificate of registration shall not be transferable.

Subregulation 3

Where a certificate of registration is lost, destroyed or defaced, the holder of the certificate may apply to the Commissioner for a replacement certificate.

Subregulation 4

The Commissioner may require an applicant to produce evidence, including a statutory declaration by the applicant, that the certificate of registration issued earlier by the Commissioner has been lost, destroyed or defaced.
