Citation and commencement
These Regulations may be cited as the National Registry of Diseases (Chronic Kidney Failure Notification) Regulations 2011 and shall come into operation on 1st March 2011.

Citation and commencement
These Regulations may be cited as the National Registry of Diseases (Chronic Kidney Failure Notification) Regulations 2011 and shall come into operation on 1st March 2011.
Definitions
In these Regulations, unless the context otherwise requires —
“chronic kidney failure”, in relation to a patient, refers to the state when —
the Glomerular Filtration Rate (GFR) (corrected to the body surface area of 1.73m2) of the patient is less than 15 ml/min;
the serum creatinine level of the patient is more than or equal to 500 µmol/L or 5.7mg/dl; or
the patient’s kidney function has deteriorated to the extent that, in the opinion of a medical practitioner responsible for the management of his care, the patient requires treatment for kidney failure,and the patient is not, in the opinion of a medical practitioner responsible for the management of his care, suffering from acute kidney failure;
“patient” means a person to whom a notification referred to in regulation 3 relates;
“treatment for chronic kidney failure” means any treatment for kidney failure administered to a patient suffering from chronic kidney failure;
“treatment for kidney failure” means any of the following modalities of treatment: (a)haemodialysis and its related modalities (including chronic haemofiltration and chronic haemodiafiltration);
peritoneal dialysis;
kidney transplantation (including follow-up medical treatment post-transplant);
conservative medical management.
In these Regulations —
any reference to the date of commencement of treatment for chronic kidney failure of a patient at a healthcare institution shall be a reference to the date on which the patient first receives any modality of such treatment at that healthcare institution; and
any reference to the date of change of modality of treatment for chronic kidney failure of a patient at a healthcare institution shall be a reference to the date on which the patient first receives a different modality of such treatment from that which he was previously receiving at that healthcare institution.
Form, time and manner of notification for chronic kidney failure
For the purposes of section 6(1) of the Act, a notification by the manager of a healthcare institution to the Registrar under that section shall —
contain, in relation to the patient, the information specified in the First Schedule;
be made in the appropriate Form or Listing specified in the Second Schedule, which Form or Listing may be downloaded from the Registry’s Internet website at http://www.nrdo.gov.sg or obtained from the office of the Registry during such hours as the office is open for business;
be sent to the Registrar —
by hand (including by courier);
by registered mail; or
by using such secure electronic notification system as may be approved by the Registrar; and
be made within the following time:
in the case of a patient diagnosed with chronic kidney failure at the healthcare institution, not later than 3 months after that diagnosis is made;
in the case of a patient who undergoes kidney transplantation at the healthcare institution, not later than 3 months after the completion of any transplant operation;
in the case of a patient who undergoes treatment for chronic kidney failure (other than kidney transplantation) at the healthcare institution —
which commenced before 1st March 2011 and continues on or after that date, not later than 31st May 2011; or
which commenced on or after 1st March 2011, not later than 3 months after the date of commencement of the treatment;
in the case of a patient who undergoes a change of modality of treatment for chronic kidney failure at the healthcare institution, not later than one month after the date of change of modality of such treatment.
Collection of additional information under section 7 of Act
Upon receiving a notification referred to in regulation 3 from the manager of a healthcare institution, the Registrar or an authorised Registry officer may, for the purposes of section 7(1) of the Act, require the manager to provide all or any of the additional information specified in the Third Schedule concerning the patient.
