Form of affidavit
The affidavit supporting a creditor’s bankruptcy application must be —
(a)
in Form PIR-4 or Form PIR-5, whichever is appropriate for the application;
(b)
filed at the same time as the creditor’s bankruptcy application; and
(c)
made by the applicant creditor or another person on the applicant creditor’s behalf.
