4. Application for licences

(1)
Every application for a licence or for renewal thereof shall be made to the Director‑General in such form as he may require.
(2)
Every application to alter the list of irradiating apparatus in respect of which an N2 licence is granted must be made to the Director‑General in such form as the Director‑General may require.
