Application for Authority’s approval
Subregulation 1
An application for the approval of the Authority for any works to be carried out on a public street shall —
be made in such form or manner as the Authority may require;
(aa)specify the period of time required for the works (including works required to reinstate the public street) to be completed;
(ab)state whether any traffic lane, or any part of any traffic lane, of the public street must be closed to traffic for the works to be carried out and, if so, specify —
the number of traffic lanes, or parts of a traffic lane, that will be closed; and
the length of each traffic lane, or part of each traffic lane, that will be closed;
contain such other information as may be required by these Regulations or by the Authority; and
be accompanied by —
the following sums:
an application fee of $165;
the estimated charge payable for carrying out works on a public street as determined in accordance with the method of calculation specified in regulation 6A;
the plans relating to the works prepared in accordance with the Code of Practice;
a certified true copy of an insurance policy taken out in the name of the applicant or the contractor appointed by the applicant which indemnifies the Authority against all claims and liabilities in respect of any injury, damage or loss which may be suffered by any person by reason of the carrying out of the works;
where the approval of any other Government or statutory authority is required in order that the works may be carried out, a copy of such approval; and
such other documents as the Authority may require.
Subregulation 2
Where the works proposed to be carried out will affect or is likely to affect the structure of any bridge, underpass, tunnel or culvert or the stability of any slope, wall or other structure, the applicant shall state that fact in his application.
Subregulation 3
The application shall be made by the following person:
where the works are required by a statutory authority, an officer of the statutory authority who has been duly authorised for that purpose; and
in any other case, by a professional engineer engaged by the person for whom the works are to be carried out.
Subregulation 4
An application under this regulation must reach the Authority at least 14 days before the date of commencement of the works proposed to be carried out.
