Regulation 11

Contents of notice of appeal

A notice of appeal —

(a)

must state —

(i)

the name and address of the appellant;

(ii)

the name and address of each authorised representative of the appellant or each legal representative of the appellant, if any; and

(iii)

an address in Singapore, and an email address (if any), for the service of documents;

(b)

must contain —

(i)

a brief description of the decision of the Authority appealed against; and

(ii)

a brief description of the grounds for appealing against the decision of the Authority and as to whether the appeal is against the whole, or only a part, of the decision of the Authority; and

(c)

must be signed and dated by —

(i)

the appellant;

(ii)

any authorised representative of the appellant, on the appellant’s behalf, if the details provided in paragraph (a)(ii) relate to one or more authorised representatives of the appellant; or

(iii)

any legal representative of the appellant, on the appellant’s behalf, if the details provided in paragraph (a)(ii) relate to one or more legal representatives of the appellant.
