4. Grant of certificate of medical need

A specified assessor may grant a certificate of medical need to an individual under section 23K of the Act by —

(a)

submitting a duly completed electronic copy of the certificate of medical need to the Authority on behalf of the individual, in the form and manner specified by the Authority; or

(b)

issuing a hardcopy of a duly completed certificate of medical need to the individual in the form specified by the Authority.
