THIRD SCHEDULERegulations 4(2)(f), 5, 16, 17, 18(2), 20(1), 22(2), 23(1), 24, 25(2) and 26Fees1.For an application for registration as an optometrist or an optician (regulation 4(1)) $50 2.For registration as an optometrist or an optician, payable at the time of registering the person (regulation 5) $200 3.For a certified copy of an entry in register (regulation 16) $50 4.For a certified true copy of a certificate of registration (regulation 17) $50 5.For registration of any additional qualification (regulation 18) $50 6.For the restoration of a registration (regulation 20(1)) $300 7.For a duplicate certificate of registration (regulation 22(2)) $50 8.For an application (regulations 23(1) and 24(1)) —
for a practising certificate or renewal thereof —
