4A. Notices to Public Guardian under section 11(7) or (8) of Act

A notice to the Public Guardian under section 11(7) or (8) of the Act must be in the form provided at the Public Guardian’s website and sent to the Public Guardian’s postal address or Public Guardian’s email address.
