Regulation 45
Unintended and accidental medical exposure
Every licensee must ensure that all practicable measures are taken to minimise the likelihood of unintended or accidental medical exposure arising from —
(a)
flaws in design of or operational failure of medical radiological equipment;
(b)
failures of or errors in software; or
(c)
human error.
