Regulation 3

Application for registration as pharmacist

Subregulation 1

The form to be used for the purpose of an application for registration as a pharmacist under the Act is that set out at the Council’s Internet website at http://www.spc.gov.sg, and every such application shall be accompanied by all of the following:

(a)

a certified copy of the degree or other qualification referred to in section 16, 17 or 19 of the Act;

(b)

a certified copy of the identity card or passport of the applicant;

(c)

a certificate of registration as a pharmacist, or equivalent, in any foreign country if the applicant has been registered in that country;

(d)

where applicable, a certificate of good standing from the registration body of a country where the applicant has been registered stating whether on the date of issue —

(i)

the applicant is a registered pharmacist of that country; and

(ii)

proceedings have been taken or are pending against him;

(e)

any other information, statement or document required by the Council; (f)the appropriate fee specified in the Schedule.

Subregulation 2

Any document produced under paragraph (1) which is not in English shall be accompanied by a certified translation thereof in English.
