Regulation 13

Subregulation 1

An instructor shall at all times keep an up-to-date register of every trainee to whom he is providing instruction in any form of martial art.

The register referred to in paragraph (1) shall contain the following personal particulars of the trainee:

(a)

name — in English and, if applicable, in Chinese characters;

(b)

NRIC No. (with official reference);

(c)

date of birth;

(d)

race;

(e)

sex;

(f)

citizenship;

(g)

residential address;

(h)

number of the certificate of registration issued to the trainee under regulation 5; and

(i)

occupation.
